The rules for travelling internationally with your pet can vary widely depending on your destination. We recommend calling the Hamilton CFIA office at 905 572 2201 to speak with a government veterinarian to confirm the requirements for your destination.

As the pet owner, it is your responsibility to understand the requirements for your destination, to provide the necessary forms and documentation, and to schedule your appointment within the requisite time frame for your destination.

As your vet, we will perform pre-travel physical examinations, complete the presented forms and documentation, and administer or prescribe vaccines, microchipping, and parasite preventives as requested. We are only responsible for the medical care of your pet - we are not responsible for knowing the specific requirements for your destination. If you have any concerns about the pet travel process (especially if you are travelling to Australia, New Zealand, China, Korea, or Japan) we recommend hiring a pet travel service to coordinate your requirements.

This form is for existing clients only. We are not currently accepting new clients for travel documentation.
