New Client Form

We are only accepting new clients on a limited basis for routine and preventative health visits at this time. We are not accepting new clients for urgent care, same-day, or emergency cases. We are typically booking 3-4 weeks in advance, and availability to take on new clients varies week-to-week depending on the needs of our existing clients.

Please fill out the new client intake form and we will review your submission. If we are able to accommodate your request, you will be contacted by email within 2 business days to schedule an appointment. Forms with incomplete or missing information will not receive a response.
